Most clinics I talk to are proud of their reminder system. Reminders go out on time, every time, automatically. And yet the no-shows keep happening, and everyone quietly concludes that no-shows are just part of running a clinic.

Here's the uncomfortable reframe: the reminder was never the job. The reply is the job. A reminder system with no follow-up isn't a system at all. It's a hope, sent by SMS.

Silence is an answer

Think about what actually happens when a reminder lands. The patient who is definitely coming replies YES in about four seconds, because it costs them nothing. The patient who isn't coming does something different: nothing. Replying to say "actually, I can't make it" takes effort and feels like a small confrontation, so they don't. They just go quiet and let the appointment fall over on its own.

Which means a patient not replying is rarely a patient who missed the message. It's a patient telling you, as softly as possible, that they're probably not going to show.

If you don't follow up on the silence, you're the one leaving a dead appointment sitting in the diary.

That's the part that stings, because the information was right there. The unconfirmed list the day before is the most accurate no-show forecast your clinic will ever have. Most clinics generate it every single day. Working through it by hand is where the hours go.

Why follow-up eats the week

To be clear, admin teams do check for replies. Good ones check constantly. The problem is what checking actually involves: reading each reply, matching it to the right appointment, updating the status somewhere, deciding who still needs chasing, then doing it all again an hour later. In a busy clinic that loop becomes almost a full-time job in itself, squeezed in between the phone and the person standing at the desk. And on the days it can't be squeezed in, tomorrow's no-shows sail through untouched.

The tooling doesn't help as much as it should. Cliniko does show the replies, but they land in a communications tab with no easy way to track what's happened to each one: which replies have been actioned, which patients are confirmed, and who has been quiet for three days. The information is there; the status isn't. So the desk ends up reconstructing the picture by hand, every time, from a list that never stops moving.

What follow-up actually looks like

The clinics that get no-shows under control all converge on the same simple ladder:

• Track replies as a status, not a message. Every appointment is confirmed, unconfirmed, or flagged. The status is visible to everyone at the desk.
• Nudge the quiet ones. Still silent a day or two after the reminder? A friendly second text, worded like a human wrote it. Most silent patients confirm within minutes of the nudge, because this time they were actually looking at their phone.
• Escalate the truly silent. Anyone still quiet by 5pm the day before gets flagged for a phone call. Now you find out tonight, not tomorrow at 2pm, and you have 18 hours to fill the slot instead of 18 minutes.

None of this is complicated. It's just hours, every day, forever. The ladder holds up in the quiet weeks and falls apart in the busy ones, which are precisely the weeks you can least afford the empty chairs.
